SGVJAA SAFETY & VOLUNTEER REQUIREMENTS

Palos Verdes Mavericks – Tackle Football & SGVJAA Sideline Cheer (Grades 5–8)

All volunteers must complete clearance requirements prior to field or sideline access.

ADULT VOLUNTEERS

(Tackle Coaches, Team Staff, ADs, Chain Crew, Field Crew, SGV Sideline Cheer Coaches & Volunteers)

Registration & Documentation

☐ SGVJAA Membership Application Completed & Signed (located in "Resources")
☐ Copy of Driver’s License / CA ID Submitted
☐ Annual SGVJAA Membership Dues Paid

Background Clearance

☐ Live Scan Completed (as required by SGVJAA)
☐ DOJ Clearance Received
☐ SGVJAA Membership Approved
☐ Membership ID Card Issued

Required Trainings (Annual)

☐ Required SGVJAA Clinics / Certifications Completed
☐ Concussion Training Completed
☐ Sudden Cardiac Arrest (SCA) Training Completed
☐ Concussion Information Sheet Signed
☐ Sudden Cardiac Arrest Information Sheet Signed
☐ Mandated Reporter Training Completed (CA AB 506 Compliant)
☐ CPR / First Aid Certification (Head Coaches & Designated Safety Leads)

Note: At least one CPR-certified adult must be present at all practices and games.

Important:
• Student Trainers must be supervised at all times by a cleared adult coach.
• Live Scan is not required for minor student trainers.
• Adult (18+) trainers must complete full Adult Volunteer clearance
