SGVJAA Certification Requirements

Athletes participating in SGVJAA conference programs must also submit additional certification documents prior to the official Certification Day. All documents can be found in the "Resources" section. These include:

Participant Contract

• Signed by parent/guardian and athlete
• Includes parent code of conduct and concussion acknowledgment

* Must print and sign (labeled SGVJAA 2026 Participant Contract)

Physical Examination Form

• Completed and signed by a licensed physician
• Physician must certify athlete is cleared for participation
• Doctor’s license number and stamp required

* Print and sign, must complete physical  AFTER May 1, 2026 (labeled SGVJAA 2026 Physical Form)

Player Liability Waiver and Insurance Information

• Completed and signed by parent/guardian and athlete
• Proof of insurance required

* Must print and sign (labeled SGVJAA 2026 Waiver)

Proof of Age

• Birth Certificate or Conference-issued Player ID Card

* Original birth certificate or passport will ned to represented for certification (8/1/26), no copies

Proof of Residence

• Three documents verifying address

School Report Card

• Most recent report card required

Emergency Information Form

• Must be completed and maintained by team staff

*Print and sign

These documents are required by the San Gabriel Valley Junior All-American Football Conference (SGVJAA) for certification eligibility.
