PLAYER REGISTRATION FORMS FOR 2009

For the initial registration,  all that is required is the Players Contract (Registration Form) and  the appropriate fee. Cheer/Dance require one check, while Football requires two separate checks - one for registration and one for equipment deposit. (See Registration Page for fee structure.)  Simply click on the link below and the Players Contract(Registration Form) will open.  PRINT IT OUT, complete, sign and bring it to registration. Your General Manager will ask for the remaining forms.

In order to participate in our program, each player or cheerleader will ultimately be required to complete the following forms or supply the following information:

 

    • Players Registration Form (Code of Conduct is on page two)
    • Copy of a current utility bill
    • Physical Form
    • Report Card for the entire previous year
      (for the 2009 season, that means the report card for the 2008-09 academic year)

    Parents: Be aware the the School offices close soon after the last day of classes. Make sure you have the needed documentation PRIOR to the summer break. As soon as you receive the final report card, please reduce and combine into a single document, prior to submitting any requested copies to your GM.

 

FOOTBALL AND CHEER COACHES FOR 2009

Palos Verdes Youth Football & Cheer  is now accepting applications for the 2009 season.  We are looking for a few good men and women to work with the youth of our community. The process of identifying and selecting Head and Assistant Coaches occurs yearly.  There is no automatic carry-over of coaching positions from one year to the next.  However, Head Coaches from the past year will receive first consideration for any post.  The process is as follows:
  • Interested Head Coaches and Assistant Coaches complete an application that signifies their interest.  This form is due on April 23, 2009.
  • Head Coaches are selected by the Executive Board of Directors and confirmed by May 10th, 2009.
  • Assistant Coaches  are then recommended by the Head Coach of each team or squad and presented to the Executive Board of Directors for approval.
  • Head and Assistant Coaches must provide a copy of their Driver's License,  complete a Volunteer Form, get a Live Scan, and receive clearance  from the State of California as a result of the Live Scan before they  are allowed to coach any team. 
  • Each Coach will be provided a badge granting them access to the game fields.

 COACHING AND ADULT VOLUNTEER  APPLICATION

Additionally, for adults desiring to be a head coach, in either cheer or football, these applications are due  April 23, 2009.  Please click on the documents below, print them, and forward the completed application via US Mail to the Association address listed below. 

Assistant Coach applications are due by June 5, 2009 to the Association.  Assistant coaches are chosen by the respective Head Coach of each team and their name is then brought before the Executive Board for approval.  The Board will not place assistant coaches on a coaching staff, but reserves the right to remove from consideration an assistant coach.

Coaching in our organization is a privilege, not an automatic right.  Feedback from parents is taken seriously as consideration is given to the comments we received each year.

Other adult volunteer positions (General Manager, City Commissioner, photographer, etc.) also use this form.  For those positions, this form is completed and supplied to the team General Manager by July 21, 2009.  They will be processed with the certification paperwork.

Background Checking Documents

Our new association with Pop Warner requires that ALL adult Volunteers will need to submit to a background check this year. If you have previously been Live Scanned, you will still need to submit this new form to PVF&C. Your prior approval was only good while we were associated with the PCC.
